Ignore the $15 (page 1) and $10 (page 3) ones because you have to buy beer with it!
Scroll down the screen from the link provided above, use the $10 (page 2) and $7.50 (page 4) ones without beer purchase. Check on the bottom to check if your state is included. If you see Utah on it, these are the exact ones I'm talking about. Print them. You can do two of them per household.
